Weeks 1–2
Pain
Calm the pain down.
Hands-on work settles the neck and shoulder muscles that have been holding you up all day, starting your very first visit.
What We Treat
The 3pm ache between your shoulder blades. The shoulders you keep telling yourself to pull back. Posture isn’t willpower, it’s endurance, and it starts with a cause-first evaluation, not a guess.

Why It Happens
You already know how to sit up straight. Holding it for eight hours is the part nobody trained you for. It’s usually some mix of four culprits: mid-back segments that got stiff and stopped extending. Deep neck and shoulder blade muscles with no staying power, so bigger muscles take over and get sore doing it. Tight hips and a chair that tilt your whole base backward. And a screen, a keyboard, and a phone that pull you into the same shape hundreds of times a day.
None of that shows up on a quick glance. It takes a real, cause-first evaluation to sort out which one is driving your symptoms, because a stiff joint and a fatigued muscle need opposite things. That’s why a brace, a new chair, or a reminder to sit up so often falls short: they answer the shape, not the source. You’re the one who holds everything together for everybody, back to back, all day. Nobody has looked at what that’s costing your neck and shoulders yet. That’s what the evaluation is for.
The Honest Truth
Nobody regrets starting early. The waiting is the part that costs you.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for posture.
Weeks 1–2
Pain
Hands-on work settles the neck and shoulder muscles that have been holding you up all day, starting your very first visit.
Weeks 2–6
Prime
Targeted mobility work gives your mid-back and shoulders back the extension they quietly stopped offering.
Weeks 6–10
Perform
Progressive deep neck, shoulder blade, and trunk strengthening builds the staying power you were missing in the first place.
Weeks 10+
Prevent
Your desk setup, your lifting and carrying, and a full workday get tested before you’re cleared: that’s the endpoint, not just another appointment.
Every visit here is one-on-one with a Doctor of Physical Therapy. Your evaluation looks for the cause of the shape you keep fighting, whether that is a stiff joint, a muscle with no staying power, or a workday that never lets up. Then you get a plan with an endpoint.

Phase one: settle the neck and shoulders that have been covering for everything else
Phase four: upright through a full workday, without thinking about it
Questions, Answered
Yes, though not by remembering to sit up straight. Posture is mostly two things: how much motion your joints have, and how long your muscles can hold you there. Free up the mid-back segments that stopped extending, then build endurance in the muscles that hold the position, and upright stops costing you effort. That’s exactly what the four phases do, in order.
They can make a day more comfortable, and neither one builds anything. A brace holds you in a shape your muscles still can’t hold on their own. A better chair changes your position without changing the joint that got stiff. Sort out the mobility and the endurance first, and then a good setup has something real to support.
That depends on what the evaluation finds, and you won’t be left guessing about it. At your first visit a Doctor of Physical Therapy maps your plan across all four phases, Pain, Prime, Perform, and Prevent, then tells you where you’re starting and what has to happen to finish. You get a plan with an endpoint: you’ll always know which phase you’re in and what’s left, not an open-ended string of appointments.
