Weeks 1–2
Pain
Calm the pain down.
Hands-on work takes pressure off the irritated nerve and settles the muscles guarding around it, starting your very first visit.
What We Treat
The first ten steps out of bed. The burn that runs down the back of one leg and decides what kind of day this is. It’s fixable, and it starts with a cause-first evaluation, not a guess.

Why It Happens
Your back doesn’t hurt for no reason. And it’s almost never just one thing. It’s usually some mix of four culprits: a disc that’s irritating a nerve root and firing pain down the leg. Spinal joints that got stiff and stopped sharing the load. Deep stabilizers and glutes that quietly weakened after your last flare-up. And the daily loading, how you sit, bend, and lift, that keeps reopening the same spot before it can settle.
None of that shows up on a quick glance. It takes a real, cause-first evaluation to sort out which one is driving your pain, and whether the leg symptoms are coming from the nerve or from the hip. That’s why rest, ice, or a generic stretching sheet so often falls short: they answer the symptom, not the source. You’re the one who loads the car, carries the laundry up two flights, and never mentions it. Which is exactly why your back has been last on the list, and exactly why it doesn’t have to stay there.
The Honest Truth
Nobody regrets starting early. The waiting is the part that costs you.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for a back and a sciatic nerve.
Weeks 1–2
Pain
Hands-on work takes pressure off the irritated nerve and settles the muscles guarding around it, starting your very first visit.
Weeks 2–6
Prime
Targeted mobility work restores motion in the stiff spinal segments and hips that have been guarding since the first flare-up.
Weeks 6–10
Perform
Progressive core, glute, and hip strengthening builds the support your spine was missing in the first place.
Weeks 10+
Prevent
Lifting, bending, and twisting get tested under real load before you’re cleared to go all-in: that’s the endpoint, not just another appointment.
I love my physical therapist, Rachel Richard. She has helped me with several issues: a frozen shoulder, pelvic floor problems and hip and back pain. Rachel is thorough and she really listens. Rachel has also shared all kinds of information that helped me on my journey to recovery.

Phase one: take the pressure off the nerve and calm the back down
Phase four: lifting and bending tested under real load, then you’re done
Questions, Answered
Usually not, and it’s rarely the first step. Back and leg pain often settle without imaging, and scans routinely show disc bulges and wear and tear in people who have no pain at all, so a picture doesn’t always point at the cause. We start with a hands-on evaluation to find what’s actually driving your symptoms, and if imaging ever becomes genuinely necessary, we’ll help you get it. You don’t have to wait on a scan to start feeling better.
Yes, in most cases movement helps more than rest does. Hurt doesn’t always mean harm, and total rest tends to leave a back stiffer and weaker, which sets up the next flare-up. The key is dosing the right movement for your back and your nerve symptoms, which is exactly what a Doctor of Physical Therapy sorts out at your evaluation, then progresses safely as you improve.
That depends on what the evaluation finds, and you won’t be left guessing about it. At your first visit a Doctor of Physical Therapy maps your plan across all four phases, Pain, Prime, Perform, and Prevent, then tells you where you’re starting and what has to happen to finish. You get a plan with an endpoint: you’ll always know which phase you’re in and what’s left, not an open-ended string of appointments.
