Why It Happens

The X-ray is the diagnosis. It isn’t the whole story.

How much an arthritic joint hurts on a given Tuesday is driven by more than what the picture shows. It’s usually some mix of four things: motion you’ve slowly given up, so the joint now lives in a smaller and smaller arc. Muscle around the joint that weakened while you were protecting it, leaving the joint to carry load on its own. A boom and bust pattern, everything on the good days and nothing on the sore ones. And the joints above and below quietly changing how you load the painful one.

Every one of those is changeable. That’s the part people rarely get told, because nobody sat down and worked out which of them applies to you. A cause-first evaluation does exactly that. The diagnosis stays. How much it limits your week is a different question. Wear on a scan and pain in a joint don’t line up as neatly as people assume, and imaging often shows changes in joints that feel fine. What tends to move is the strength around the joint, the motion it has, and how load is spread across your day. Those are the things we go after, which is why the first step is a cause-first evaluation and not another picture.

Joints do better with movement than without it, and resting an arthritic joint tends to leave it stiffer and the muscle around it weaker, which makes the next flare easier to trigger.
